Requests for Missing or Replacement CEU Certificates
Please check your junk/spam folders before requesting certificates. You will find your certificate email there 99.9% of the time. If it is not there, AOTA requires that we make positive identification of the recipient before reissuing certificates. Please provide the following information:

  • Your full name if you are not submitting your request from an email address that displays your name or your name has changed (e.g., email uses maiden name but certificate was issued using your married name)
  • Title of webinar completed
  • Approximate date of webinar post-test
  • Whether you attended via GoToTraining or view-on-demand
  • Purchase receipts and post-test documents, if available
